Medical Records
The Medical Records Department is open Monday through Friday from 8:00 a.m. to 5:00 p.m.
To obtain copies of your medical records, you must complete an Authorization to Release Medical Records form. This form is available in the Medical Records Department, at any Reception area and on this web site under “Patient Forms.” Please allow 7 to 10 days to process your request.
