Rules of eligibility

No purchase is necessary.
MyHealth applications must be received by the MyHealth Coordinator no later than 9:00 p.m. on Wednesday, November 30, 2011.
MyHealth accounts must be activated by 9:00 p.m. on December 2, 2011 by:

creating your MyHealth User ID & Password
accepting the terms and conditions for you and any family member linked to your account

All patients with an active MyHealth account are eligible. (If you already have a MyHealth account, you’re automatically entered into the drawing.)
The drawing will be held on Monday, December 5, 2011 in South Shore Medical Center’s Administration Department.
Winners’ names will not be made public at the time of the drawing for confidentiality purposes.
Winners will be contacted by telephone immediately following the drawing.

MyHealth is a great tool for our patients, but to get the most out of it, you might find it helpful to attend one of our free Introduction to MyHealth classes. If you have enrolled in MyHealth, join us to learn more about this useful tool.

Please join us Monday, December 5th in our Norwell offices. There will be a class at 10:00 a.m. and another at 7:00 p.m. It will be fun and easy!
