South Shore Medical Center is pleased to offer a Diabetes Passport incentive program for our patients who are living with Diabetes.*
Speak with your Primary Care Provider about our DIABETES PASSPORT checklist to help you stay healthy with Diabetes in 2011:
- Have your dilated retinal eye exam.
- Office visits with weight and blood pressure check twice a year, and foot exam once a year
- Lab appointment at least twice a year, including two HgA1c (a long term measure of your blood sugar), one LDL (“lousy”/ bad cholesterol), and one microalbumin (a test for your kidneys).
Diabetes is the leading cause of blindness, kidney disease/failure and amputations. Decreasing your HgA1c by 1% toward the goal of 7% will reduce your risk of eye and kidney complications by 35%!! (ADA – UKPDS study results)
Prevention is the key, there are everyday things you can do to help control your diabetes:
- Follow a healthy meal plan.
- Become more physically active.
- Maintain proper oral hygiene.
- Refrain from smoking.
- Monitor feet for cuts, blisters, and sores.
- Take diabetes medication as needed.
- Monitor blood glucose
If you are getting your diabetes care with another provider outside of South Shore Medical Center, you are eligible to participate in our Diabetes Passport Program. However, it is very important that you provide us with copies of your lab work, eye exam, foot exam, etc so that you may be eligible for this program.
*Patients who have a completed passport by December 31, 2011 will be entered into a raffle for a chance to win one of the twenty $250 supermarket/pharmacy gift cards. Additionally, patients who are able to achieve one or more of the Hemoglobin A1c, Blood Pressure, LDL goals will receive two additional entries into the raffle. Winners will be announced Jan 31, 2012. Each person is only eligible to win one gift card.
