MyHealth Online is a highly secure, web-based program that allows you to contact your doctor’s office about non-urgent medical matters.

Please log in using the button on the left. If you are a new user who has received an access code from us, you need to activate your account before you can log in for the first time.

If you are interested in using MyHealth, or need to activate your account, select the "Sign Up and Activate" tab above.

Signing up for MyHealth is easy!

To sign up, fill out an online application. You will receive an e-mail with an access code to complete the process. Once you have been notified that the processing of your request is complete, you may begin using MyHealth. Be sure to provide an e-mail address that you check regularly.

MyHealth Security

South Shore Medical Center is committed to ensuring the confidentiality of all patient information. With MyHealth, all communications between you and your physician’s office are carried over a secure, encrypted connection. To prohibit unauthorized access, all medical information is stored safely behind our firewall.

In addition:
• You set your own username and password.
• Your password is known only to you.
• MyHealth automatically logs you out after 15 minutes of inactivity.

Adult Proxy Access

Proxy access allows you to use MyHealth Online to communicate with the doctor’s office of a family member or loved one regarding non-urgent matters. Access is granted once a signed paper authorization form has been completed and processed by the patient’s primary care site location. Download Adult Proxy Application (37k pdf).

Adolescent Access

Adolescent access for MyHealth Online allows an adolescent patient between the ages of 13 to 18 to view
components of their medical record and communicate with their physician’s office regarding non-urgent matters. The parent/legal guardian will also be able to view a limited portion of the adolescent’s medical record. In order to obtain access, both the parent/legal guardian and the adolescent must complete and sign the Adolescent Access Authorization Form. Adolescent Access is terminated when the patient or parent/legal guardian makes a written or online request to terminate access. At age 18 the patient will have full control of their MyHealth Online account and the parent/legal guardian access will be discontinued. Beyond the age of 18 the patient may still grant parent/legal guardian access by completing a MyHealth Online Adult Proxy Access form. Download Adolescent Access Form (41k)

Pediatric Proxy Access

Pediatric Proxy for MyHealth Online allows you to securely communicate on behalf of a pediatric patient between the ages of newborn and 13 years of age. Access is automatically terminated on child's 13th birthday. Download Pediatric Proxy Authorization Form (43k)

MyHealth is an online patient portal that provides a password-protected, secure area on the South Shore Medical Center website. With MyHealth you may view lab results and check certain portions of your health record. In addition, you may send a message to your doctor’s office, request an appointment or a prescription renewal and complete other tasks.

MyHealth makes it easier for you to communicate with your doctor's office and monitor your health records online. All transactions are secure and private.

With MyHeath you may view routine lab results, immunizations, appointments, check upcoming appointments, and view active medications.

Request Prescription Renewal

It may be difficult to find the time in your busy day to request a prescription renewal by telephone. You may log on to MyHealth anytime to request that a prescription be renewed.

MyHealth is not intended to replace an office visit. It should only be used for routine and non-urgent matters.

If you need immediate assistance, call your physician’s office.

You may request to schedule a non-urgent appointment through MyHealth. Let us know days and times that are convenient for you. If an appointment time is available, we will schedule it or notify you of other available times.
