Appointments

SSMC sees patients by appointment only, both for routine and urgent care visits. Please call and speak with the secretaries between 8:30 a.m. and 4:30 p.m. to book routine appointments, or speak with the nurse to schedule an urgent care appointment. You may also request an appointment through the on line patient form.

Please bring your insurance card with you and any co-payment that is required at every visit. Payment for services not covered by insurance is due at the time of the visit. Check with your insurance company for coverage information.

NOTE: Your health plan may have special requirements regarding the use of hospital or specialized services. Please acquaint yourself with your plan before you need specialized or emergency care.
